We are also providers for the Traditional Blue Cross Blue Shield Program, Kaiser Permanente, PacificSource, Aetna, Vantage MCO, Providence Preferred Oregon, Healthnet, Innovative Care Management, Ethix, Selectcare, Good Health Plan, ODS Health Plan, Lifewise, Providence Medical Group and Tuality Medical Supply. All Medicare, Medicaid, and Oregon Health Plan members are welcome as well as Washington State Department of Social and Health Services members. If your insurance company is not listed, please contact us. We are in the process of becoming providers for several different insurance companies.
